Scientific studies show that although stroke is primarily a focal lesion, it also affects the functional connectivity of anatomical and functional regions, often resulting in altered integration of brain networks and affecting whole-brain function, leading to cognitive and emotional impairment [2, 3].</p> <p><bold><italic>AIM: </italic></bold>The aim of the study was to determine changes in functional brain connectivity during hypersomnia in patients with acute ischemic stroke.</p> <p><italic><bold>MATERIALS AND METHODS:</bold> </italic>A total of 44 patients with acute ischemic stroke were examined. The participants were divided into two groups based on the presence of sleep disorders. Group 1 included 22 patients with hypersomnia, which was objectively confirmed by polysomnography. Group 2 also included 22 patients who did not have sleep disorders and constituted the control group. The age of patients in both groups ranged from 45 to 65 years.</p> <p>All patients underwent magnetic resonance imaging on tomographs with a magnetic field induction strength of 1.5 Tesla, using the standard protocol and special pulse sequences of T-gradient echo 3D MPRAGE and BOLD. Resting-state functional magnetic resonance imaging of the brain was employed to assess functional connectivity. Postprocessing was conducted on specialized software, CONN-TOOLBOX, which generated appropriate graphical representations of quantitative results based on the selection of zones of interest.</p> <p><italic><bold>RESULTS:</bold> </italic>In patients experiencing the acute phase of ischemic stroke, hypersomnia results in the strengthening of functional connections, predominantly in the temporo-occipital and parietal regions. This may be associated with impaired visual perception, memory, and spatial orientation. Additionally, there is a weakening of functional connections in the frontal and occipital cortex, which may indicate confusion of thinking and disorders of speech, arbitrary movements, and the regulation of complex behaviors.</p> <p>The disruption of the functional connections between the medial prefrontal cortex and the posterior cingulate cortex and the cerebellum is indicative of impaired coordination and regulation of balance and muscle tone. However, it also has the potential to affect emotional, cognitive, and behavioral changes in the brain.</p> <p><bold><italic>CONCLUSIONS: </italic></bold>Resting-state functional magnetic resonance imaging is a technique that allows for the determination of changes in functional brain connections during hypersomnia in patients with acute ischemic stroke. Additionally, it enables the identification of neuroimaging markers corresponding to this pathology.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Поражение головного мозга после ишемического инсульта приводит к изменениям в широком спектре структурных и функциональных сетей мозга [1]. Научные исследования демонстрируют, что, хотя инсульт в основном является очаговым поражением, он также влияет на функциональную связность анатомических и функциональных областей, что часто приводит к изменению интеграции сетей мозга и влияет на функцию всего мозга, вызывая когнитивные и эмоциональные расстройства [2, 3].</p> <p><bold>Цель </bold>— определить изменения функциональных связей головного мозга при гиперсомнии у пациентов с острым ишемическим инсультом.</p> <p><bold>Материалы и методы.</bold> Обследовано 44 пациента с острым ишемическим инсультом. Участники были разделены на две группы в зависимости от наличия нарушений сна. В первую группу вошли 22 пациента с гиперсомнией, которая была объективно подтверждена при помощи полисомнографии. Во вторую группу вошли также 22 пациента, которые не имели нарушений сна и составили группу контроля. Возраст пациентов обеих групп составлял от 45 до 65 лет.</p> <p>Всем пациентам была проведена магнитно-резонансная томография на томографах с силой индукции магнитного поля 1,5 Тл, с применением стандартного протокола и специальных импульсных последовательностей Т-градиентного эхо 3D MPRAGE и BOLD. Для оценки функциональных связей использовали функциональную магнитно-резонансную томографию головного мозга в состоянии покоя. Постпроцессинговую обработку проводили на специализированном программном обеспечении CONN-TOOLBOX с соответствующим графическим представлением количественных результатов на основе выбора зон интереса.</p> <p><bold>Результаты.</bold> У пациентов в остром периоде ишемического инсульта при гиперсомнии определяется усиление функциональных связей преимущественно в височно-затылочных и теменных отделах, что может быть связано с нарушением зрительного восприятия, памяти и пространственной ориентации. Кроме того, наблюдается ослабление функциональных связей в лобной и затылочной отделах коры головного мозга, что может свидетельствовать о спутанности мышления и нарушениях речи, произвольных движений и регуляции сложных форм поведения.</p> <p>Нарушение функциональных связей медиальной префронтальной коры и задней поясной извилины с мозжечком говорят не только о нарушениях координации и регуляции равновесия и мышечного тонуса, но также могут широко влиять на эмоциональные, когнитивные и поведенческие изменения в мозге</p> <p><bold>Заключение. </bold>Методика функциональной магнитно-резонансной томографии в состоянии покоя позволяет определить изменения функциональных связей головного мозга при гиперсомнии у пациентов с острым ишемическим инсультом и выявить нейровизуализационные маркёры, соответствующие данной патологии.</p></trans-abstract><trans-abstract xml:lang="zh"><p/></trans-abstract><kwd-group xml:lang="en"><kwd>sleep disorders</kwd><kwd>hypersomnia</kwd><kwd>acute ischemic stroke</kwd><kwd>functional magnetic resonance imaging</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>нарушения сна</kwd><kwd>гиперсомния</kwd><kwd>острый ишемический инсульт</kwd><kwd>функциональная магнитно-резонансная томография</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><citation-alternatives><mixed-citation xml:lang="en">Salvalaggio A, De Filippo De Grazia M, Zorzi M, Thiebaut de Schotten MT, Corbetta M.
