Whole-body magnetic resonance imaging (WB-MRI) is a noninvasive, non-ionizing imaging technique that provides comprehensive information on skeletal and extraskeletal metastatic lesions, structured using the MET-RADS-P (METastasis Reporting and Data System for Prostate Cancer) and enabling detailed assessment of treatment response. However, the use of this system has not been widely adopted, and its diagnostic accuracy requires clinical validation.</p> <p><bold>AIM: </bold>This study aimed to evaluate the interim efficacy of <sup>225</sup>Ac-PSMA radioligand therapy using WB-MRI based on MET-RADS-P criteria in patients with metastatic castration-resistant prostate cancer, assess method reproducibility (intra-observer agreement), and compare imaging findings with changes in prostate-specific antigen (PSA) levels.</p> <p><bold>METHODS: </bold>WB-MRI was performed twice in patients with metastatic castration-resistant prostate cancer: before administration of <sup>225</sup>Ac-PSMA-617 with activity 6–12 MBq and 1–2 months after a single cycle of radioligand therapy, with parallel measurement of PSA levels. Prospective response assessment was conducted using MET-RADS-P criteria for WB-MRI and PCWG criteria for PSA level trends. Reproducibility of MET-RADS-P criteria was evaluated through repeated reading of WB-MRI studies by the same radiologist with a 6-month interval.</p> <p><bold>RESULTS: </bold>After one cycle of radioligand therapy, MET-RADS-P progression was identified in 4 of 20 patients with metastatic castration-resistant prostate cancer (20%) who completed the study. In three of these patients, progression was detected at an earlier time point than indicated by PSA level trends. More than half of the patients demonstrated discordant responses, including three with progression. Differences in PSA level trends were observed between concordant positive responses and various patterns of discordant response according to MET-RADS-P criteria. In cases of concordant response, the MET-RADS-P integrated assessment showed good correlation with biochemical response. MET-RADS-P criteria demonstrated excellent reproducibility for the primary response category pattern and for integrated response assessment. Reproducibility for the secondary response category pattern was substantial.</p> <p><bold>CONCLUSION: </bold>The use of MET-RADS-P criteria to WB-MRI in patients with metastatic castration-resistant prostate cancer enabled earlier detection of progression in skeletal and extraskeletal lesions compared with PSA level trends and demonstrated good intra-observer agreement. Further investigation of the prognostic value of MET-RADS-P criteria in the context of <sup>225</sup>Ac-PSMA radioligand therapy is warranted.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>Радиолигандная терапия препаратом, представляющим лиганд к простатоспецифичному мембранному антигену, меченный актинием-225 (<sup>225</sup>Ас-ПСМА) — новый перспективный метод лечения метастатического кастрационнорезистентного рака простаты. Магнитно-резонансная томография всего тела (МРТ ВТ) — неинвазивный неионизирующий метод лучевой диагностики, предоставляющий информацию о скелетных и внескелетных метастатических очагах опухоли, структурированную с помощью системы MET-RADS-P (METastasis Reporting And Data System for Prostate Cancer) и включающую детальную оценку результата лечения. Однако использование данной системы не является общепринятым, а её диагностическая эффективность нуждается в клинической проверке.</p> <p><bold>Цель исследования. </bold>Оценить промежуточную эффективность радиолигандной терапии препаратом <sup>225</sup>Ас-ПСМА с помощью МРТ ВТ с учётом критериев MET-RADS-P у пациентов с метастатическим кастрационно-резистентным раком простаты, проанализировать воспроизводимость метода (внутриэкспертное согласие) и сопоставить результаты с динамикой концентрации простатоспецифичного антигена (ПСА).</p> <p><bold>Методы. </bold>МРТ ВТ пациентам с метастатическим кастрационно-резистентным раком простаты проводили дважды: перед введением препарата <sup>225</sup>Ас-ПСМА-617 активностью от 6 до 12 МБк и через 1–2 мес. после одного курса радиолигандной терапии с параллельным определением концентрации ПСА. Проспективную оценку ответа на радиолигандную терапию проводили с учётом критериев MET-RADS-P для МРТ ВТ и критериев PCWG (Prostate Cancer Working Group) для оценки динамики концентрации ПСА. Воспроизводимость критериев MET-RADS-P определяли путём повторного чтения результатов МРТ ВТ с интервалом 6 мес. одним и тем же рентгенологом.</p> <p><bold>Результаты. </bold>После одного курса радиолигандной терапии из 20 пациентов с метастатическим кастрационно-резистентным раком простаты, завершивших исследование, прогрессирование по критериям MET-RADS-P выявлено у четырёх (20%), из них у троих — на более ранней временной точке, чем в соответствии с динамикой концентрации ПСА. Более половины пациентов имели несогласованный ответ, в том числе трое — с прогрессированием. Выявлены различия в динамике концентрации ПСА при однонаправленном положительном и различных вариантах несогласованного ответа по критериям MET-RADS-P. При однонаправленном ответе интегральная оценка по MET-RADS-P хорошо коррелировала с биохимическим ответом. Критерии MET-RADS-P имели превосходную воспроизводимость для первичного паттерна категории оценки ответа и для интегрального ответа. Для вторичного паттерна категории оценки ответа воспроизводимость была существенной.</p> <p><bold>Заключение. </bold>Критерии MET-RADS-P при оценке МРТ ВТ у пациентов с метастатическим кастрационно-резистентным раком простаты позволили выявить прогрессирование скелетных и внескелетных очагов на более ранней временной точке, чем динамика концентрации ПСА и имели хорошее внутриэкспертное согласие. Целесообразно дальнейшее изучение прогностической значимости критериев MET-RADS-P при радиолигандной терапии препаратом <sup>225</sup>Ас-ПСМА.</p></trans-abstract><trans-abstract xml:lang="zh"><p><bold>论证：</bold>以锕-225（<sup>225</sup>Ac）标记的前列腺特异性膜抗原（prostate-specific membrane antigen, PSMA）放射性配体治疗是一种用于转移性去势抵抗性前列腺癌的新型且前景广阔的治疗方法。全身磁共振成像（whole-body MRI，WB-MRI）是一种无创、无电离辐射的影像学检查方法，可获取骨及骨外转移灶的信息，并借助MET-RADS-P（METastasis Reporting And Data System for Prostate Cancer）体系实现结果的结构化呈现和对治疗反应的详细评估。然而，该评估体系尚未被普遍采用，其诊断效能仍需临床验证。</p> <p><bold>目的：</bold>评估基于MET-RADS-P标准的WB-MRI在<sup>225</sup>Ac-PSMA放射性配体治疗中期疗效评估中的应用价值；分析该方法的可重复性（同一阅片者一致性），并将影像学结果与前列腺特异性抗原（prostate-specific antigen, PSA）浓度变化进行比较。</p> <p><bold>方法：</bold>对转移性去势抵抗性前列腺癌患者进行两次WB-MRI检查：第一次在注射活度为6–12 MBq 的<sup>225</sup>Ac-PSMA-617之前完成，第二次在完成一次放射性配体治疗后1–2个月进行，并同步评估PSA浓度的动态变化。放射性配体治疗的前瞻性疗效评估采用WB-MRI的MET-RADS-P标准，并依据PCWG标准对PSA动态变化进行评估。通过同一名放射科医师在间隔6个月后对WB-MRI结果进行重复阅片，评估MET-RADS-P标准的可重复性。</p> <p><bold>结果：</bold>在完成研究的20例患者中，单次放射性配体治疗后，根据MET-RADS-P标准，有4例（20%）被判定为疾病进展，其中3例的影像学进展时间早于PSA浓度变化所提示的进展。超过一半的患者表现为不一致的反应，其中包括3例进展。在MET-RADS-P标准下，单向阳性反应与不同类型的不一致反应在PSA动态变化方面存在差异。在单向反应情况下，MET-RADS-P的综合评估与生化反应呈良好相关性。MET-RADS-P在主要反应评分类别及综合反应评估方面表现出极高的同一阅片者一致性。在次要反应评分类别中，其一致性为显著水平。</p> <p><bold>结论：</bold>在转移性去势抵抗性前列腺癌患者中，基于 MET-RADS-P标准的WB-MRI可较PSA浓度动态更早识别骨及骨外转移灶的疾病进展，并具有良好的同一阅片者一致性。有必要进一步研究MET-RADS-P标准在225Ac-PSMA放射性配体治疗中的预后意义。</p></trans-abstract><kwd-group xml:lang="en"><kwd>whole-body magnetic resonance imaging</kwd><kwd>metastatic castration-resistant prostate cancer</kwd><kwd>MET-RADS-P criteria</kwd><kwd>radioligand therapy</kwd><kwd>225Ac-prostate-specific membrane antigen</kwd></kwd-group><kwd-group xml:lang="ru"><kwd>магнитно-резонансная томография всего тела</kwd><kwd>метастатический кастрационно-резистентный рак простаты</kwd><kwd>критерии MET-RADS-P</kwd><kwd>радиолигандная терапия</kwd><kwd>225Ас-ПСМА</kwd></kwd-group><kwd-group xml:lang="zh"><kwd>全身磁共振成像</kwd><kwd>转移性去势抵抗性前列腺癌</kwd><kwd>MET-RADS-P标准</kwd><kwd>放射性配体治疗</kwd><kwd>225Ac-PSMA</kwd></kwd-group><funding-group/></article-meta></front><body></body><back><ref-list><ref id="B1"><label>1.</label><mixed-citation>Nosov DA, Volkova MI, Gladkov OA, et al.
